RN Case Manager, Per Diem

Description

     The Case Manager is responsible and accountable for patient trajectory by assisting the healthcare team in pacing care toward improved patient outcomes.

The Case Manager role is structured around four major functions: Care Coordination, Utilization Management, Discharge Planning and Preventing Unnecessary Re-admissions.

Each of these functions supports the Care Management department’s goals:
• Improved patient satisfaction and patient outcomes.
• Decreased length of stay.
• Decreased unit cost.

Case Managers will provide case management resource and support to the Med/Surg Units, Admitting/ Registration, Pre-admit testing, SDC, and the inpatient setting

Performs the six essential activities of Case Management: Assessment, Planning, Implementation, Coordinate, Monitor and Reassess through the continuum of care to facilitate a safe cost-effective transition post discharge.

     Schedule:  Per Diem, weekend and holiday rotation.

Qualifications

BSN required. Minimum of 5 years of broad clinical experience, predominately in medical/ surgical nursing, home care or post acute setting. Case management, discharge planning or utilization review experience preferred. Graduate of an NLN-accredited School of Nursing. CCM preferred. Current RN license in the Commonwealth of Massachusetts. Ability to collaborate with other disciplines both in and out of the network. Ability to maintain relationships with staff, outside agencies and payers, patients and families of a diverse and multicultural population. Ability to multi task and work in a fast paced environment. High level organization skills required.
