Clinical Social Worker, LICSW

Clinical Social Worker provides clinical services to patients seeking treatment in the Dual Diagnosis Partial Hospital Program and other affiliated programs by providing education, individual and group therapy, and diagnosing and developing a treatment plan for patients with addiction and psychiatric co-morbid conditions. Ensures Joint Commission, CMS, DPH, DMH and BWH/BWFH compliance.

Responsible for the following for patients admitted to the Dual Diagnosis Partial Hospital Program:        

Completion of an inital assessment

Formulation of an intial treatment plan

Provide detailed orientation to the program

Completion of the treatment consent

Provision of education, individual and group treatment, and case management services to patients and their families

Meet with patients for weekly individual psychotherapy as deemed necessary

Development of a clinical discharge plan for continuing care 

Discharge planning

Refer patients to the appropriate level of care as needed

Verification of treatment letters

Completion of all required daily progress notes, discharge planning, group planning, billing and other documentation as necessary 

Obtain medication authorizations

Provide clinical coverage for affiliated programs in the Addiction Recovery Program as requested by the supervisor

Participate in research or quality improvement projects as requested by the supervisor

Assisting in the education of trainees (e.g. medical students, psychiatry residents) as requested by the supervisor

Vacation coverage to other Psychiatry/Addictions areas as necessary for safe patient care

Attendance at all staff and treatment team meetings

Assist in research and quality improvement projects as requested

Staff adheres to all I.C.A.R.E. Standards

Ability to write/formulate a psychosocial assessment.

Ability to use diagnostic criteria based on the DSM IV/5 to formulate a diagnostic impression and determine level of functioning

Academic and working knowledge of addiction/addictive behavior and implications in a clinical practice

Outstanding problem solving skills

Excellent verbal and telephone skills

Time management, ability to organize and manage multiple priortities and take initiative

Strong team player

Ability to manage confidential information

Computer skills including Microsoft Office (Word, Excel, PowerPoint) and Outlook

The Outpatient Department is an intensive clinical environment.  Focus:  Individual treatment, family work, consultations nads utilization review with insurance companies for continued care authorizations.  Patient/social worker ratio is variable and dependent on patient census and acuity.
